Below is a form that you need to fill out to place your extra apparel order.
Please refer to the Extra Apparel Catalog/Sheets handed out at the Parent's meeting for apparel item numbers and sizes. Check Website for "Order Deadline Date".

Reminder: All players receive 2 practice t-shirts Red Long Sleeve T-shirt & Black Short Sleeve T-shirt). Please note, these are item numbers 1 & 2 on the Apparel Catalog. Unless you want an extra t-shirt, please do NOT fill in #1 & 2 on your order form.
